Maintained by civil authorities, they are prime sources of genealogical information; but, unfortunately, official vital records are available only for relatively recent periods. These records, despite their recent creation in the United States, are critically important in genealogical research, often supplying details on family members well back into the nineteenth century. A certified copy of a birth certificate cannot be issued without proper purpose. You may obtain a birth certificate for yourself, your spouse, your parent, or your child.
Before proceeding, ensure that you meet the above proper purpose criteria (a request by the individual, parent, or spouse for the birth certificate). The statutory file search fee will be retained in the event that no record is located or the proper person has not requested the record. You will be notified in writing if no record is located or proper purpose criteria has not been met.
To receive a birth certificate, send a $8.00 check or money order and the following information:
- Full name at birth. If adopted, list adopted name and state “adopted”.
- Month/day/year of birth
- City or town of birth/County of birth
- Father’s full name, if adopted, list adopted father’s name.
- Mother’s full maiden name, if adopted, list adoptive mother’s name.
